Here's what happens next:
 

1. Our team will add your information to our Electronic Medical Record (EMR) system. This typically takes place within 2–3 weeks of submission.
    

2. You do not need to call our office to confirm receipt. If your form was submitted successfully, you will see this confirmation page, and no further action is needed on your end at this time.
    

3. We will contact you directly to book your intake appointment with Dr. Ike. Appointments will begin later this summer, and we are scheduling patients in the order forms are received.
    

4. Please allow time for scheduling. Given the volume of interest in Dr. Ike's practice, it may take some time before you are contacted. We appreciate your patience.
    

What if I need to update my information or submitted the form in error?
Please email us at info@bleamsmedical.ca rather than calling, and our team will assist you.
 

What if I have an urgent medical concern in the meantime?
This intake process is for establishing ongoing care with Dr. Ike and does not apply to urgent needs. If you have an urgent medical concern, please contact your current provider, visit a walk-in clinic, or, for emergencies, call 911 or go to your nearest emergency department.
