After-Hours Clinic
Welcome to the Bleams Medical After-Hours Clinic
Our after-hours clinics are available only for rostered patients only. If you are a rostered patient of Dr. Dwivedi or Dr. Qayyum, you have access to this service for urgent medical problems during weekday evenings and weekends.
We are part of a Family Health Organization (FHO) which is a group of doctors in Kitchener-Waterloo that take turns seeing FHO patients with urgent problems after hours.
By using our after-hours services, you get:
✔ Access to a trusted team who has your full medical history
✔ Continuity of care, with updates shared directly with your family doctor
✔ No need to visit outside walk-in clinics, which can lead to fragmented care
How Do You Access the After-Hours Clinic?
Call First — Do Not Walk In Without Calling
All after-hours clinic (AHC) visits must be pre-booked by phone. Please do not show up at the clinic without calling ahead as the location of after-hours clinic varies.
To access after-hours care:

Call our office at 519-742-4321 (or 519-904-2663) after 4:00 PM on weekdays.

Listen carefully to the voicemail announcement for instructions on how to reach the on-call family doctor or clinic.
When Are After-Hours Clinics Available?
We offer after-hours services at the following times:

Monday to Thursday: 5:00 PM – 8:00 PM

Friday: 1:00 PM – 5:00 PM OR

Saturday & Sunday: 9:00 AM – 12:00 PM
Important:

These visits are for urgent concerns only — not for routine matters.

Please arrive early; clinics may stop accepting patients before the posted end time if capacity is reached.
What Counts as an Urgent Medical Problem?
The after-hours clinic is meant only for medical problems that cannot wait until our office reopens.
Examples of urgent concerns:

Sudden or worsening illness (fever, infections, sore throat, minor injuries)

Urgent prescription needs (e.g., running out of essential medication)

Acute symptoms needing timely medical evaluation
Not appropriate for after-hours clinics:

Routine prescription refills

Forms or paperwork

Follow-up on non-urgent issues
WHY CHOOSE AFTER-HOURS CLINICS OVER WALK-IN CLINICS?
We strongly discourage our patients from visiting walk-in clinics for several reasons:

The FHO doctors running after-hours clinics can access your medical records to ensure they are familiar with your medical history, which leads to better comprehensive care.

The FHO doctors also share your visit records with your family physician, ensuring continuity of care.

Walk-in clinics typically don't share your visit records with your primary doctor which may disrupt your health care management.

Your family doctor will not know what happened at the walk-in clinic, what tests were ordered, or what treatment/referrals were done.

Your family physician may be charged by the Ministry of Health for the cost of your walk-in visit.

Repeated walk-in clinic visits could lead to de-rostering from your family doctor's FHO and the loss of after-hours services.
