Dr. Fareeha QayyuM, MD, CCFP.

 

Dr. Qayyum completed her medical degree in Hamilton at McMaster University followed by Family Medicine training at the University of Toronto.

Dr. Qayyum has been practicing family medicine in the Region of Waterloo for over 10 years with a strong focus on elderly population, chronic disease management and preventive care.

Dr. Pooja Dwivedi, MBBS, CCFP. ​​

 

Dr. Pooja Dwivedi provides comprehensive family medicine with a strong focus on older adults, geriatric care and the long term management of chronic conditions. Her background as a general practitioner in the United Kingdom has shaped her into a steady and thoughtful clinician who approaches care in a very organized and practical way.
 

She has a warm and friendly personality, and patients often feel comfortable speaking with her about their health concerns. Her clinical work includes ongoing management of high blood pressure, diabetes, cholesterol and age related health conditions.She places importance on regular monitoring and ensuring that patients understand the key points they need to follow between visits.
 

Dr. Dwivedi is well acquainted with the healthcare community across Kitchener, Waterloo and Cambridge. She understands the local referral pathways and connects patients with the appropriate specialists, diagnostic services and community programs when additional support is needed.
 

She is fully licensed to practise family medicine in Ontario and continues to build her patient panel at our clinic. Patients who would like to join her practice are welcome to add their names to our waiting list.
