Clinic Policies 

Booking Appointments:

To book an appointment, please call our office at 519-742-4321 or 519-904-2663.

Regular appointments are scheduled for 10 minutes. Considering the time allotted for regular appointments, it is recommended to address only 1-2 concerns each visit. This ensures that your family doctor can appropriately address your concern in a safe and comprehensive manner. 

 

Additional medical concerns will need to be addressed at a separate appointment.

Same Day Appointments: 

We offer same day appointments for your semi-urgent health care needs. For urgent matters, please visit the nearest emergency department. 

 

After Hours Evening and Weekend Services: 

After-hours care for urgent medical problems is available in the evenings on the weekdays, and on the weekends through our call group of family doctors. The clinic times for the after-hours care for Monday to Thursday are 5:00 pm to 8:00 pm.

Location will vary depending on the physician on-call. Friday to Sunday - Clinic times will vary depending on the physician on-call. Please call our office (519-742-4321) after-hours for a message regarding the specific timings for the weekend on-call clinic. Please, note that the use of the after-hours service is intended for matters of urgent medical nature that cannot wait until our office reopens.


 

Cancellation and Late Arrival for Appointments: 
Please call the office and leave a message at least 24 hours in advance if you need to cancel your appointment.
If you miss your appointment without providing 24 hours notice, you will be charged a missed appointment fee (see list of non-OHIP or uninsured services). If you arrive more than 15 minutes late for your appointment, your appointment will be considered missed, and you will be asked to reschedule for another date. We work hard to keep our clinic running on time, and we appreciate when you arrive early for your appointments. 

Test Results: 
Your family doctor will review all your test results that they order (eg, blood work, x-rays, etc.), and you will be called for a follow-up appointment to review your results if your doctor deems necessary.

You may also book your own follow-up appointments to review and discuss any test result.

Please do not call the office asking for your results, as our support staff are not trained to provide test results over the phone.

Forms: 
Completion of forms is a service not covered by OHIP, and you may be billed an appropriate fee for form completion (see list of non-OHIP or uninsured services). In general, forms will take 2-4 weeks to complete. You must make an appointment to review your forms with a physician or nurse.

Bleams Medical Centre is committed to providing a safe, secure, and respectful environment for all patients and staff. Abusive behaviour, words or actions that make others feel threatened or demeaned will not be tolerated and may result in dismissal from our practice.

Non-OHIP or Uninsured services 

What are "non-OHIP" covered services or "uninsured services"?


Non-OHIP covered services are those services provided to patients that are not covered by the Ontario Health Insurance Plan (OHIP). The Ministry covers all insured medically necessary services provided by physicians, however, it does not pay for services that are not medically necessary. These may include: writing sick notes, cosmetic procedures, charges for missed appointments, completing forms for daycare, insurance, legal or employment purposes, or if your health card is not valid on the day of your appointment.
 

Why am I being charged for these services?


OHIP pays physicians for most medical services delivered to patients. Services not covered by OHIP or another insurance plan are called uninsured services. Your physician will request a payment from you for those services. It is the patient's responsibility to pay for uninsured services.

How are the uninsured fee rates determined?


When fees are charged for uninsured services, they are based on the Ontario Medical Association's suggested fees as found in the most current edition of the OMA Physician's Guide to Third Party and Other Uninsured Services.
