• We will soon be accepting new patients. Visit our new patients page and fill out the waitlist form. 

• For booking appointments or any other matter, please call our office at 519-742-4321 and our staff will be able to assist you. You can also book non-urgent appointments through our website's online appointment portal. Click here to book an appointment online.

  • Phones are answered on Mon-Tue and Thur 9:30am – noon, 1:30pm – 4pm and Wed and Fri 9:30am – noon.
     

• For prescription renewals, please ask your pharmacy to fax us the prescription renewal request at 519-742-1305. Please ensure you call AT LEAST 2 weeks before you will run out of your regular medications.

• Completion of government, work-related medical and insurance forms may take up to 2-4 weeks.
   

• For after-hours evenings and weekends on-call clinic, please call our office at 519-742-4321 and listen carefully to the announcement where we provide contact information of the after-hours clinic for that evening or weekends. Outside these hours, please call Telehealth Ontario at 1-866-553-7205.
   

• Please note that we are an appointment only clinic. This is NOT a walk-in clinic.
