Annual Block Fee Plan
Some services provided by our clinic are not covered by OHIP. These may include certain medical notes, forms, and prescription renewals without an appointment, when appropriate.
Our optional annual plan lets you pay one fee for the uninsured services included in the plan during the coverage period. You may also choose to pay for individual services as needed.
Annual coverage for one adult
Annual coverage for you and your spouse or partner.
Annual coverage for one adult aged 65 or older.
Annual coverage for two spouses or partners, both aged 65 or older.
Annual coverage for your family, including eligible dependent children and full-time students (under 21)

Cash or cheque dropped off at our office is our preferred payment method. You can also call us to arrange payment.

What does the plan include?
Included services cover a range of uninsured clinical and administrative requests, such as:

Prescription renewals without an appointment, when clinically appropriate.

Certain illness, return-to-work, school, and camp notes or forms.

Certain referral notes and insurance forms.

Selected administrative services, such as replacement documents and transfer of medical records.

Driver’s medical forms receive a 50% discount under the annual plan. Missed appointment fees and legal reports are not covered.
Common questions
Do I have to join?
No. The annual plan is optional. You can pay for individual uninsured services as needed.
Does the plan cover every service?
No. Please review the fee guide before enrolling. Some services are excluded or discounted rather than fully covered.
Does the plan guarantee prescription renewals without a visit?
No. Renewals depend on clinical appropriateness. Your physician may require an appointment to reassess your condition.
How long does coverage last?
This plan covers December 1, 2026, through November 30, 2027.
