      • Mon-Tue and Thur 9:30am – noon, 1:30pm – 4pm

      • Wed and Fri 9:30am – noon.

    • ​Note: Our phone lines are busy during peak hours. If you are unable to get through, please leave a voicemail during clinic hours and a team member will return your call within 24 hours.

    • Please note that we are an appointment only clinic. This is NOT a walk-in clinic.

  • After-Hours Clinic Timings:

  • For prescription renewals, please ask your pharmacy to fax us the prescription renewal request at 519-742-1305. Please ensure you call AT LEAST 2 weeks before you will run out of your regular medications.
     

  • Completion of government, work-related medical and insurance forms may take up to 2-4 weeks.

After Hours Clinic (AHC)

after hours clinic IS AVAILABLE for Dr. Qayyum'S and Dr. Dwivedi's patients

We are part of a Family Health Organization (FHO) which is a group of doctors in Kitchener-Waterloo that take turns seeing FHO patients with urgent problems after hours.

If you are a rostered (registered) patient of Dr. Dwivedi's and Dr. Qayyum's, you can access after-hours medical care for urgent medical problems during weekday evenings and on the weekends through our call group of on-call family doctors. 


Usual hours of operation of these after hours clinics are: 
 

AFTER HOURS CLINIC
Monday to Thursday: 5:00pm-7:30pm 
Friday: 1:00pm- 5:00pm (Hours may vary on Fridays)
Saturday and Sunday: 9:00am - 12:00pm
Location: Varies depending on the physician on call

Outside these hours, please call Telehealth Ontario at 1-866-553-7205.

How do you access the AFTER HOURS CLINIC?
All AHC appointments are booked by calling in first. DO NOT show up to the clinic without calling. If calling any after hours clinics, please call our office (519-742-4321) after 4pm only and listen to our announcement.  
 
AFTER HOURS CLINIC is only for URGENT MEDICAL PROBLEMS
Please, note that the use of the after-hours service is intended for matters of urgent medical nature that cannot wait until our office reopens. 
 

WHY CHOOSE AFTER-HOURS CLINICS OVER
WALK-IN CLINICS?
 

We strongly discourage our patients from visiting walk-in clinics for several reasons: 
​

  • The FHO doctors running after-hours clinics can access your medical records to ensure they are familiar with your medical history, which leads to better comprehensive care.

     

  • The FHO doctors also share your visit records with your family physician, ensuring continuity of care.
     

  • Walk-in clinics typically don't share your visit records with your primary doctor which may disrupt your health care management. 
     

  • Your family doctor will not know what happened at the walk-in clinic, what tests were ordered, or what treatment/referrals were done. 
     

  • Your family physician may be charged by the Ministry of Health for the cost of your walk-in visit.
     

  • Repeated walk-in clinic visits could lead to de-rostering from your family doctor's practice and the loss of after-hours services. 

EMERGENCY SERVICES

If you are having severe symptoms (e.g. severe pain, vomiting blood, chest pain, significant shortness of breath) please do NOT come to the after hours clinic but instead call 911 or go to your nearest emergency department.
