For booking appointments or any other matter, please call our office at 519-904-2663 and our staff will be able to assist you.

Phones are answered on Mon-Tue and Thur 9:30am – noon, 1:30pm – 4pm and Wed and Fri 9:30am – noon.

For prescription renewals, please ask your pharmacy to fax us the prescription renewal request at 519-742-1305. Please ensure you call AT LEAST 2 weeks before you will run out of your regular medications.

Completion of government, work-related medical and insurance forms may take up to 2-4 weeks.

For after-hours evenings and weekends on-call clinic, please call our office at 519-742-4321 and listen carefully to the announcement where we provide contact information of the after-hours clinic for that evening or weekends. Outside these hours, please call Telehealth Ontario at 1-866-553-7205.

After-hours care for urgent medical problems is available in the evenings on the weekdays, and on the weekends through our call group of family doctors.

The clinic times for the after-hours care for Monday to Thursday are 5:00 pm to 7:30 pm. Location will vary depending on the physician on-call. Friday to Sunday - Clinic times will vary depending on the physician on-call.

Please, call our office (519-904-2663) after hours for a message regarding the specific timings for the weekend on-call clinic and the number for contacting the after-hours clinic. Please, note that the use of the after-hours service is intended for matters of urgent medical nature that cannot wait until our office reopens.
